Please arrive 10 – 15 minutes early for your first dental appointment to complete our electronic New Patient Forms, or if you prefer, click the link below and fill them out in advance. Please be sure to select the appropriate link (child or adult) based on the age of the patient (children 0-17 in age).

During your first appointment, you’ll meet with Dr. Sheils and discuss your dental and health history and situation and talk about your goals, what you like and don’t like about your smile, and what you’d like to change in your dental health.

We’ll complete a thorough examination, and we may take X-rays, conduct other diagnostics, make molds and discuss dental care recommendations and options, including a cost estimate. Lastly, we will put you on our schedule for your treatment!
